What Are Headers and Footers in PDF? Bates Numbering and Page Labels

PDF headers and footers display information at the top and bottom of every page — page numbers, dates, document titles. Learn how they're implemented and about Bates numbering.

Headers and footers in PDF are typically content in the page's content stream positioned in the top and bottom margins — not a special PDF feature, just text drawing operators like any other content. Alternatively, some PDF workflows implement headers and footers as page-level annotations or as shared Form XObjects referenced from every page. The most notable specialized header/footer feature in PDF is Bates numbering, used in legal and litigation document management.

How Headers and Footers Are Stored

Unlike Word, PDF has no dedicated "header" and "footer" zones. A PDF created by Word includes the header/footer text drawn in the page content stream at specific Y coordinates. It looks like a header because it's positioned in the top margin; structurally it's identical to any other text on the page. Some PDF tools implement headers/footers as annotations (which can be separately toggled/deleted) or as Form XObjects (a shared content definition referenced from each page's content stream, allowing a single change to update all pages). The approach used depends on the PDF creation tool.

Adding Headers and Footers to Existing PDFs

In Adobe Acrobat Pro: Edit → Header & Footer → Add. You can add text (including variable fields: page number, total pages, creation date, custom text) at top-left, top-center, top-right, bottom-left, bottom-center, or bottom-right. Headers and footers added this way are stored as overlaid content appended to each page's content stream. They can be updated ("Update") or removed ("Remove") via the same menu. Page range and page number format options are available.

Bates Numbering

Bates numbering is a sequential document numbering system used in legal, medical, and litigation document management to uniquely identify every page in a document production. Format: typically a prefix + sequential number, e.g., "ABC00001", "ABC00002". Bates numbers allow attorneys to reference specific pages precisely: "defendant's exhibit, ABC00047" identifies exactly one page in potentially hundreds of thousands of produced documents. In Acrobat Pro, Bates numbering is added via Tools → Edit PDF → Header & Footer → Add Bates Numbering. It can span multiple documents, with numbering continuing across the entire set.

PDF Page Labels

PDF page labels are a separate feature from visual page numbers in headers/footers. Page labels define how pages are labeled in the PDF viewer's page count display and navigation (the current page indicator). A document might have: Roman numeral pages (i, ii, iii) for a preface, then Arabic numerals (1, 2, 3) for the body, then back to Roman numerals for appendices. Page labels are stored in the Catalog's PageLabels number tree and control how viewers display page identifiers — not the visual content on the page.
